FMC Policy 4023- When scanning in documents into a patients chart it is very important that the document is scanned into the correct category.
Example: Florida Medical Clinic’s new patient paper work (Patient Registration Forms, HIPAA, PHI, Consent) must be scanned in as follows:
  o Category: Admin
  o Description: MM/DD/YYYY (Date signed) FMC New Patient Registration Forms.

Example: New Patient Questionnaires should be scanned in as follows:
